Important Luminaria Request 

This year’s Luminaria Ceremony will feature a slide show to honor survivors and to memorialize those who have lost their fight against cancer. If you would like to submit a digital photo, please e-mail it no later than Friday, February 8th to Elissa Goldstein, at kscope18@yahoo.com. 

If you only have a hard-copy photo, please make arrangements to scan it and send it electronically (if you can’t do this at home, you can scan your photo at Walgreens or CVS, save it to a CD and then e-mail it).

Please be sure to include the name of the person in the photo you submit, and indicate whether he or she is a survivor or someone who lost the fight.  Please note that we consider those currently undergoing treatment to be survivors). If possible, please submit a photo of just the individual being honored/memorialized (if you don’t have one and/or can’t crop your photo, a group photo will be accepted).

Please be sure to upload your photo in a JPEG format no smaller than a 720x486 size.

Important Forms for Relay

• Relay 101:  this is an important document filled with information specific to our relay. 

•  Spirit Points:  This document will tell you everything you need to know to win the Most Spirited Team at relay.  

• Youth Participation Waiver  any youth 18 years old or younger must turn in a Youth Agreement to their Chaperone no later than February 15th!  This form must be signed by the youth and a parent/guardian.  Each participating youth in addition to turning in this form will need to register as a participant on the team web-site.  NO FORMS WILL BE ACCEPTED AT RELAY!  If you do not have your signed form submitted to their Chaperon by February 15th will not be able to participate in Relay.

• Chaperone Agreement  any team that have youth the age of 18 or under must have at least 1 adult (25 years or older) per 5 youth.  Each Chaperone will need to submit their form no later than February 15th!  Please turn in this form with all of the Youth Participation Agreements from your team at the same time. 

• Luminaria Forms  For  those who are uncomfortable using the online tool to purchase luminaria, you may use this form.  Be sure to include your name somewhere on the form to ensure you get credit for the sale
